There are never enough staff in a medical practice, physician or non-clinical, to handle all the work to be done — from reviewing patients’ test results, to calling patients to discuss them, to answering patient calls about scheduling and clinical questions, to calling patients to check if they’re taking meds rights. Simbie is the clinically intelligent AI voice agent that automates and expands all of this work, saving practices time and money while adding patient touchpoints that save lives, but that practices can’t afford human staff for. We're the first platform executing AI workflows requiring real clinical judgment, directly in the EHR.
We're YC-backed, distribution is proven, and we're signing customers faster than we can currently bring them live. That's the problem we need you to solve.
About the Role
Every new customer means taking our AI agents from handshake to production — wired into their EHR, validated against their clinical workflows, and running reliably from day one. Right now that path is too slow for the rate we're signing. We're hiring a Product Engineer to own and accelerate it.
You'll work across the full stack but anchor in infrastructure: the deployment pipeline, EHR integrations, multi-tenant systems, and the observability that tells us our agents are doing the right thing in production. This isn't a pure platform or SRE role — you'll be close to customers and clinical workflows, while building the systems that let us launch the next customer in days instead of weeks.
You're a great fit if you want to turn a high-touch, founder-driven onboarding process into repeatable, self-serve infrastructure — and own scale and reliability of the product.
What You'll Do
- Own the path from signed customer to live in production: provisioning, EHR integration, configuration, and go-live, then make that path faster and more repeatable every time.
- Build and harden integrations with EHR systems (FHIR, HL7, vendor APIs) so new customers connect with less custom work.
- Design multi-tenant infrastructure that lets us onboard and isolate customers safely and at speed.
- Build the deploy, CI/CD, and infrastructure-as-code that make launches fast and rollbacks safe.
- Instrument agent behavior in production — logging, metrics, tracing, and alerting — so we know our AI is making the right clinical judgments, and we catch it when it doesn't.
- Ship full-stack product features end-to-end alongside the infra work; the two aren't separate here.
- Own reliability for systems doing patient-facing clinical work, including on-call and turning incidents into permanent fixes.
- Build internal tooling so onboarding a customer stops requiring an engineer in the loop.
What We're Looking For
- 3+ years building and operating production software as a full-stack or backend engineer.
- Strong with Python and React and able to ship a full-stack feature on your own.
- Experience building and operating integrations against third-party or partner APIs.
- Solid grasp of observability and debugging in production: logs, metrics, traces, and the instincts to use them.
- A bias toward shipping and a high tolerance for the ambiguity of an early-stage, fast-moving environment.
Nice to Have
- Experience with healthcare data and standards (FHIR, HL7, EHR integrations) — or eagerness to go deep fast.
- Familiarity with HIPAA, SOC 2, PHI handling, and building under compliance constraints.
- Experience taking a product through rapid customer growth, where onboarding velocity was the bottleneck.
- Background building internal platforms or self-serve tooling that removed engineers from a manual loop.
- Experience deploying or operating LLM-based / agentic systems in production.
