Craniometrix Hits $4 Million ARR and Raises $15 Million Series A to Scale Dementia Care Navigation
Craniometrix has raised a $15 million Series A round after reaching $4 million in annual recurring revenue. The New York-based healthtech startup launched its dementia care navigation service in July 2025 and has now raised more than $21 million in total funding. Backers include Y Combinator, Quiet Capital, Defy.vc, Rebel Fund, Olive Tree Capital, Cathexis Ventures, and J-Ventures.
The company operates with 36 employees and is hiring across software engineering, operations, and regional healthcare partnerships to expand its coverage nationwide.
Virtual Support Through Medicare's GUIDE Model
Craniometrix provides virtual, non-clinical coaching and support to dementia patients and their families. The company runs its service in partnership with the Johns Hopkins MIND at Home program, aiming to help families manage care routines and keep patients living at home safely.
The business model connects directly to Medicare's Guiding an Improved Dementia Experience (GUIDE) model. Through this program, Medicare pays for caregiver coaching and care coordination, allowing Craniometrix to deliver services to patients at no direct cost while generating a new revenue stream for the patients' primary care physicians. According to the company, providing structured non-clinical coaching and support helps avoid 30% of hospitalizations for dementia patients. Craniometrix deploys artificial intelligence systems to support care navigators and ease administrative overhead for clinical providers.
Evolution from Diagnostic Testing
Craniometrix was founded in 2022 and went through Y Combinator's W22 batch. The company originally launched with a focus on early cognitive assessment. As reported by TechCrunch, the team raised an initial $6 million seed round to build a HIPAA-compliant smartphone app designed to test for Alzheimer's disease using a 10-minute mobile test. A physician reviewed and signed each diagnostic result before returning it to the patient.
Forbes noted that early studies showed the smartphone-based diagnostic method demonstrated higher accuracy than existing conventional screening tools, leading to co-founder Nikhil Patel's inclusion on the 2023 30 Under 30 Healthcare list. Over the past three years, the company expanded from diagnostic software into ongoing longitudinal care management, leveraging federal reimbursement frameworks to offer continuous support services.
Leadership
Craniometrix is led by co-founders Nikhil Patel and Neel Patel.
Nikhil Patel serves as CEO and has published academic research on digital diagnostics for Alzheimer's disease. Before founding Craniometrix, he spent three years during high school working as a research associate at the University of Central Florida studying Alzheimer's diagnostics. He completed a BS in Computer Science and a BA in Economics at Yale University between 2017 and 2021, and spent time developing trading algorithms at Goldman Sachs and working at hedge funds before starting the company.
Co-founder and CEO Neel Patel manages operational execution as the company expands its clinical and provider relationships under Medicare's billing structures.
