About Zócalo Health

Zócalo Health is a virtual family medicine service designed for the Latino patient. We remove barriers to healthcare by offering same-day appointments and providing care based on building relationships rather than completing transactions. Each member of Zócalo Health is paired with a community health worker (promotora de salud) to establish a relationship-based partnership with a focus on understanding the member’s goals and values. Research findings have shown that relationship-based partnerships, over time, have the power to influence health outcomes. At the core of this care model is a focus on prevention, primary care, behavioral health, and traditional practices that work together-in relationship- to support our member’s wellness.
