About this role

The Pharmacy Student Clinical Support Specialist is a unique, remote clinical position designed for Ohio-based pharmacy students pursuing careers in ambulatory pharmacy practice. It presents an opportunity for students to sharpen their clinical knowledge and familiarity with management strategies for common ambulatory care conditions within a human-centered, population-conscious primary care-supporting organization. In this role, you will serve as the "front door" for an interdisciplinary care team, initiating deep-dive chart reviews to help inform interdisciplinary care for rising risk patients. Reviews will support the actions of Waymark community health worker (CHW), therapist and pharmacy teams in supporting Medicaid patients. 

Working under the oversight and mentorship of the market Ambulatory Clinical Pharmacist lead, you will organize and summarize Electronic Health Record (EHR) information to support the clinical pharmacist’s development of care recommendations for an interdisciplinary care team. You will be responsible for synthesizing complex medical histories into concise summaries that guide care team priorities. The role is remote and offers flexibility, with the ability to complete work during evenings or weekends, making it an ideal opportunity to complement formal pharmacy education during the academic school year. The expected duration of the role is 6-8 months. Time spent in the role does not qualify as internship hours needed to satisfy pharmacy licensure requirements in Ohio. The role requires a regular weekly commitment of at least 12 hours, with potential opportunities for increased work hours and responsibilities with the approval of Waymark’s pharmacist (max work opportunity of 19 hours per week). In addition to the routine work responsibilities outlined above, individuals will have the opportunity to develop and deliver clinical trainings for non-licensed team members as part of their professional development.

Key Responsibilities

  • Medication Record Review: review patient medical records to summarize currently prescribed medications and accurate fill histories.  This review is limited to data gathering and summarizing; the student does not perform clinical assessments, make therapeutic recommendations, or interact with patients. 
  • Care Team Mapping: Identify and document the patient’s active ambulatory care providers and specialists to ensure seamless communication.
  • ADT Event Tracking: Summarize recent Admission, Discharge, and Transfer (ADT) events to identify transitions of care needs.
  • Clinical Synthesis: Extract and identify patient’s chronic conditions and key provider recommendations.
  • Preliminary Screening: Flag medical conditions or social determinants of health that may be effectively impacted by the interdisciplinary team for pharmacist review and prioritization.
  • Clinical Documentation: Provide clear, concise written summaries of findings to be utilized by the clinical pharmacist and licensed clinical managers to direct patient care.

Minimum Qualifications

  • Education: Completion of at least 2 years (P2) of an ACPE-accredited Doctor of Pharmacy (PharmD) program or equivalent.
  • Medication Knowledge: Familiarity with most commonly prescribed medications (i.e. top 200 drugs)
  • Clinical Knowledge: Familiarity with the traditional management and pharmacological treatment of common chronic conditions (e.g., Diabetes, Hypertension, COPD).
  • Technical Skills: Proficiency in navigating complex database structures and maintaining HIPAA compliance in a remote work environment.
  • Communication: Strong clinical writing skills with the ability to summarize complex data for a multi-professional audience.
  • Commitment: Ability to commit to 12 hours per week throughout the academic year.
  • EHR Experience: Prior experience navigating Epic Electronic Health Record systems.

Preferred Qualifications

  • Education: Completion of 3 years (P3) of an ACPE-accredited Doctor of Pharmacy (PharmD) program or equivalent
  • Detail Oriented: Proven ability to spot discrepancies in medication lists and transition-of-care documentation.
  • Medication Reconciliation Experience: History of experience with medication histories
  • Population Experience: Experience serving vulnerable populations with a skill set including motivational interviewing and/or harm reduction

Hiring Range

$22.38/hr - $25.42/hr
