WHAT YOU’LL DO
As a LPN Clinical Liaison, you will be a critical member of the clinical care team, providing both clinical and administrative support to ensure high-quality, efficient member care. This role assists providers by accurately documenting patient encounters in Thyme box and supported electronic health record (EHR), supporting patient flow, coordinating care activities, and maintaining complete and accurate medical records. The LPN Clinical Liaison helps improve clinical workflow while ensuring timely communication and continuity of care.
WHAT YOU’VE DONE
Within your first three months, you will:
Complete training and be up to speed on Thyme Care’s systems, tools, technology, partners, and clinical expectations.
Maintain patient confidentiality in accordance with HIPAA regulations.
Accurately document patient/member encounters after visits—within Thyme Box and other supported EHRs, capturing histories, assessments, treatment plans, orders, diagnostic results, referrals, prescriptions, and follow-up instructions.
Organize patient charts and gather relevant clinical information ahead of visits, including vital signs, medication reconciliation, and other required data.
Review member eligibility for services and verify insurance/coverage.
Facilitate referrals, follow-up appointments, and diagnostic testing and keep care plan information updated to ensure continuity of care.
Conduct chart reviews across multiple electronic platforms to consolidate and streamline transitions of care information.
Monitor shared clinical inboxes and electronic communications, triaging and routing messages promptly to the appropriate provider, department, or team. Coordinate and communicate with internal and external healthcare partners to support seamless and coordinated care.
WHAT LEADS TO SUCCESS

Experience. You have at least 2 years as a LPN, especially with telehealth experience. It would be exceptional if you have worked at a startup or tech-forward company.
Licensure. You have a compact LPN license
Organized. You’re skilled in juggling multiple tasks and working under pressure without sacrificing organization in your communications and documentation.
Effective listener and communicator. You are winsome and articulate, but you always start with listening and hearing what may not be voiced because you listen intently to others. You build rapport and excellent working relationships with members and colleagues.
Comfort with ambiguity. Start-ups are fast-paced environments, and you understand that rapid changes to the business, strategy, organization, and priorities are par for the course… and part of the adventure.
A desire to learn how to use new technologies. We are a technology company focused on interacting with folks during the season when they need it most. Experience with video chatting, Google Suite, Slack, electronic health records, or comfort in using and learning new technology is vital.
Identify priorities and take action. You know how to identify and prioritize a member's needs and do what it takes to address urgent needs immediately.
