About the Role
We believe that dementia is too often diagnosed late– well after decline has begun– and families receive insufficient support navigating that process. That’s why we are building on the GUIDE model to diagnose dementia and facilitate care navigation earlier so that families can be better supported through a patient’s cognitive decline.
As our first full time nurse practitioner hire, you will take the lead to meet patients and families wherever they are in the dementia diagnosis journey, often initiating the first, second and third steps in the diagnostic evaluation. You will support the care navigation team in medical escalations that arise. You will work closely with Tembo’s Head of Medical to refine and develop workflows for streamlined diagnosis and support for patients with dementia, pioneering a new specialty care model focused on this population.
What You'll Do
Conduct virtual assessments of patients with suspected dementia diagnoses, including baseline cognitive assessments and further laboratory testing orders as needed
Conduct medication reviews of patients with dementia, supporting patients and their medical care team with recommendations to reduce high-risk medications and to start medications that may help their symptoms where indicated.
Develop comprehensive care plans for patients in collaboration with care navigators.
Coordinate care alongside care navigators and patient’s primary care teams including external care providers, specialists, etc. to support the patient and their caregiver(s).
Support other clinical needs that arise, including urgent care or other geriatric practice needs as the practice evolves
What Success Looks Like
30 days: You're set up to see patients in our electronic medical record system (Athena Health) and you have successfully completed some initial dementia evaluations independently. You are embedded in our multidisciplinary huddles and shadow our care navigators and head of medical in their work.
60 days: You’ve learned more of the nuances of dementia diagnosis and management. As you get more comfortable using Tembo’s systems, you have started to advise care navigators in the clinical needs of our patients.
90 days: You’re working with Tembo’s head of medical to onboard your future colleagues, learning from your experience to continue to improve and scale our programs.
About You
You love to innovate and learn; building on existing protocols to develop your own practice is exciting to you.
You thrive in the implementation of new technologies. You may have been a superuser in past technical implementations you have done before, or the person that others turned to because you “got it” a little faster.
You enjoy helping patients and families navigate the healthcare system despite its complexity.
You love working on an interdisciplinary team with care navigators/coordinators, social workers, doctors, etc.
You are comfortable working from home embedded in an all-remote team.
You’re a self-starter who identifies problems and proposes solutions, but also likes to work on an interdisciplinary team and knows when to escalate when you need help.
Requirements
Family or adult-gerontology nurse practitioner with 1 year minimum experience.
Licensed in Arizona, Oregon or Nevada with the ability to be licensed to practice virtual/outpatient care in any state.
Must not be excluded from participating in any state or federal healthcare programs.
Passion for caring for patients with dementia.
Comfort with learning new technology systems.
Compensation & Logistics
$100,000-$140,000
Fully remote. Periodic in-person team gatherings. Possible travel to learn from customers and partners. You'll be working directly with leadership. This is a high-visibility, high-impact role for the right person.
