About Tembo Health

Tembo makes dementia care effortless. Today, seniors with dementia are not consistently receiving high-value, high-quality care. The journey is costly and frustrating for seniors, families, and caregivers. We partner with health plans, providers, and senior living communities to ensure that seniors with dementia get the care they need through a) a concierge care team, b) health coaching and coordination, c) on-demand virtual urgent care, and d) passive vital sign monitoring to track wellness and clinical deterioration. Concierge care teams help seniors and their caregivers get the attention and support they need at every stage in their dementia journey. Seniors and caregivers benefit from convenient access to multidisciplinary care from a dementia-specialized team including care navigators, geriatric psychiatrists, and social workers. Over the past few years, we have helped seniors with dementia navigate care and service needs to reduce costly and risky health issues (75% ER avoidance), improve patient quality of life, and reduce family stress. 95% of seniors and families who use Tembo, recommend us. Tembo currently delivers care across 11 states and is licensed across all 50 states.
