
Referral, Patient Care Coordinator
What you need
- 1–3 yrs healthcare coordination or patient intake experience
- Experience coordinating care across multiple organizations preferred
- Strong phone presence, empathy, and persuasive communication
- Sales or customer retention experience strongly preferred
- Proficient with CRMs, EHRs, Zendesk/ServiceNow
What you'll do
- Manage inbound referral workflow and initiate outreach
- Contact patients via phone, text, and email
- Use motivational interviewing to schedule evaluations
- Coordinate scheduling with appropriate psychiatrists
- Manage outbound referrals to partner organizations
As a Referral PCC, you will be the first point of contact for patients referred to Talkiatry by our trusted network of partners, and you'll also manage the outbound side of referrals - coordinating when Talkiatry patients need to be referred out to partner organizations for services outside our scope of care. This is a patient-facing role that blends administrative efficiency, sales acumen, and cross-organizational coordination to keep referrals moving smoothly in both directions.
The successful candidate must be available to work one of the following shifts:
8:00am-4:30pm EST
10:30am-7:00pm EST

You will:
Inbound Referral Outreach & Scheduling
Manage referral workflow by initiating outreach on all referrals not yet scheduled and tracking outcomes accurately
Manage a high volume of outbound calls in a fast-paced environment
Contact referred patients via phone, text, and email to explain next steps, highlight the value of care with Talkiatry, and assist with scheduling
Use motivational interviewing techniques to address concerns, build urgency, and motivate patients to schedule their psychiatric evaluations
Coordinate scheduling using internal scheduling tools and match patients with appropriate Talkiatry psychiatrists based on need, geography, and insurance
Send self-scheduling links, follow up, and troubleshoot scheduling barriers (insurance questions, availability, etc.)
Outbound Referral Coordination
Manage the outbound referral process for Talkiatry patients being referred to partner organizations for services outside Talkiatry's scope of care (e.g., higher level of care, specialized programs, community resources)
Communicate directly with patients about the outbound referral: explaining why the referral is being made, what to expect, and next steps for connecting with the receiving partner
Coordinate with the partner organization to confirm receipt of the referral, submit any required documentation, and track the patient's status through intake
Keep the Talkiatry referring provider updated on the status of the outbound referral, including confirmation of receipt, scheduling outcomes, and any barriers encountered
Follow up with patients and partners to close the loop on outbound referrals, ensuring patients don't fall through the cracks between organizations
Document all outbound referral activity accurately in internal systems to maintain a clear audit trail for the care team
You have:
1–3 years' experience in healthcare coordination, patient intake, call center, or referrals
Experience coordinating care or referrals across multiple organizations or provider networks a plus
Ability to communicate clearly, both verbally and in writing
Sales or customer retention experience strongly preferred (e.g., upselling, rebooking, or high-volume conversion roles)
Strong phone presence, empathy, and a persuasive communication style
Comfortable working with CRMs, EHRs (eClinicalWorks), ticketing systems (Zendesk/ServiceNow)
Ability to multitask and work efficiently in high-volume environments
Must be organized, responsive, and detail-oriented with strong follow-through
