A bit about this role:
Transitions of Care (TOC) is one of Devoted Medical's largest clinical programs. From admission through 30 days post-discharge, our virtual interdisciplinary teams help thousands of members get home safely and stay there, clearing the clinical, social, and logistical barriers that usually get in the way. The TOC Behavioral Health team does this work for members leaving a behavioral health level of care — a group that too often loses access because of limited community resources or barriers tied to their own behavioral health needs.

As a TOC Behavioral Health Care Coordinator, you're the person who makes sure they don't fall through. You'll connect members to in-network community providers, book the appointment, and follow through until care actually starts.

Responsibilities and Impact will include:

  • Serving as the team's "care traffic controller" — working with members and the interdisciplinary team to track clinical needs to resolution, from locating in-network behavioral health services to scheduling appointments

  • Handling telephonic outreach and support with members and provider offices, meeting every call with patience and genuine regard for the person on the other end

  • Building and maintaining relationships with community behavioral health partners, confirming in-network availability and engaging services on members' behalf

  • Providing ongoing administrative and outreach support as members transition from the hospital to longer-term care in their community

  • Preparing for and actively contributing to weekly Interdisciplinary Team Meetings, flagging high-risk members, resource gaps, and next steps

  • Spotting process problems, proposing fixes, and partnering to implement them — we're still building this model, and your input shapes it


Required skills and experience:

  • Associate's degree and 3+ years of work experience in a healthcare setting

  • Comfort multitasking on the phone — you'll be listening, talking, and typing at the same time

  • Ability to independently manage a fully remote schedule, M–F, 8:30am–5:00pm CST, MST, or PST

  • Comfort learning new technology, including electronic health records and Google Workspace tools

  • A team-player mentality and the confidence to advocate for members with internal and external stakeholders, speaking up early when you need support rather than absorbing the problem quietly


Desired skills and experience:

  • Prior experience in a behavioral health setting, with Medicare or Medicaid populations, or with complex and underserved patients

  • Bilingual (English/Spanish)

Salary Range: $22 – $27 / hour


The pay range listed for this position is the range the organization reasonably and in good faith expects to pay for this position at the time of the posting. Once the interview process begins, your talent partner will provide additional information on the compensation for the role, along with additional information on our total rewards package. The actual base salary offered will depend on a variety of factors, including the qualifications of the individual applicant for the position, years of relevant experience, specific and unique skills, level of education attained, certifications or other professional licenses held, and the location in which the applicant lives and/or from which they will be performing the job.
