Axle Health closed a $10 million Series A in May 2025, betting that the routing brains built for ride-hail and food delivery can be rebuilt for the messier job of getting a nurse to a patient's living room. F-Prime Capital led the round, with Y Combinator, Pear VC, and Lightbank participating.

The raise came after a year that the company's CEO Adam Stansell and its press materials describe in stark numbers: revenue grew tenfold in the twelve months before the round, per Business Insider, and the company reported 900% growth over fiscal 2024 in its press release. Earlier capital had been smaller and quieter: a 2021 seed, a bridge, and a $4.4 million event in February 2024 that included a 2024 seed co-led by Pear VC and TRAC VC (Home Health Care News reported the 2024 seed as $4.2 million).

Axle Health was founded in 2020 by Stansell, cofounder Connor Hailey, and several former Uber colleagues who had spent years building logistics for Uber Eats and similar on-demand services. Stansell had helped launch Uber Eats in the northeastern United States and later joined Motive, a trucking-fleet software company. The pitch is straightforward: the real-time dispatch intelligence that matches a driver to a rider in seconds, applied to home-based clinical care, where a missed visit means a missed wound dressing or blood draw.

The company originally operated as a tech-enabled home health provider itself, joined Y Combinator's Winter 2021 cohort, and scaled to all 50 states at a few million dollars in revenue. In 2023 the team shelved that services model and pivoted to selling and licensing its software to other home health providers, and the Series A is funding that pivot.

The platform is an EMR-integrated logistics layer that handles three jobs at once: smart scheduling, route optimization, and automated patient engagement. The scheduling engine matches patients with clinicians based on clinical eligibility, patient preferences, clinician license level, and cost, all updated in real time. The routing component processes millions of data points (traffic patterns, provider credentials, patient needs, and geographic constraints) to plan visits, adapt to cancellations, and reroute clinicians on the fly. The engagement layer sends automated reminders and arrival-time estimates by text or phone, and lets patients confirm appointments or leave feedback without calling a scheduler.

Axle claims the system cuts manual scheduling work by approximately 70% and lifts clinician productivity by up to 30%, per the company's press release and website. Customers now include large health systems, independent home health agencies, mobile phlebotomy providers, and high-acuity dispatch services.

The capital is earmarked for three pushes: sharpening the AI scheduling and routing algorithms, accelerating a generative AI patient engagement product that includes AI-powered voice calls, and growing the engineering and operations teams. Stansell has also signaled deeper integrations with electronic medical record systems and direct connections to medical equipment suppliers and pharmacies.

F-Prime partner Carl Byers pointed to the team's Uber pedigree as the differentiator, saying the founders applied sophisticated logistics principles to a sector that had historically lacked them. Pear VC's Mar Hershenson stressed workflow fit, arguing that most healthtech adds friction and Axle Health removes it. That combination of pedigree and integration is what the round is funding.

How the Routing Engine Works

Axle Health's founders didn't start in healthcare. Stansell, Hailey, and several former Uber colleagues launched the company in 2020 during the pandemic, aiming to build for home health the same real-time dispatch intelligence the gig economy had spent a decade refining. That lineage shows up in the platform's architecture. At the center sits a patent-pending logistics engine that ingests millions of data points (traffic patterns, provider credentials, patient needs, and geographic constraints) to optimize clinician routing in real time and predict bottlenecks before they hit.

The visit-sequencing layer runs on more than raw drive times. The engine weighs those same clinical, preference, license-level, and cost inputs, then sequences the day accordingly. That blend matters in home health, where the wrong clinician at the wrong address isn't just inefficient; it's a compliance problem. The model treats those rules as hard constraints and the speed, continuity, and cost tradeoffs as soft ones it optimizes against.

Rerouting is where the engine earns its keep. When a visit runs long, a patient cancels, or a traffic incident strands a clinician on the highway, the system rebuilds the rest of the day rather than flagging an exception for a human scheduler to fix. Axle frames this as machine-learning models layered on years of healthcare operations expertise, though the company has not publicly named the specific model architectures or training corpora behind those predictions.

The platform is delivered as EMR-integrated software, which lets it sit on top of the electronic medical records a home health agency already runs. Those same three capabilities arrive through the same workflow clinicians use for charting, a quiet but important design choice, because a routing engine that lives outside the EMR creates new tabs and manual reconciliation, and most home health staff don't have time for either. The founders have been candid that they don't view any single routing layer as the moat. "You're not going to have one provider that's going to solve the whole thing. You need an ecosystem," Stansell told Business Insider. That stance shapes how the engine is sold. Axle licenses its technology to large health systems, independent home health agencies, mobile phlebotomy providers, and high-acuity dispatch services rather than running its own home health arm. What the Productivity Numbers Mean for an Agency

The headline figures Axle publishes — up to 30% more clinician productivity and roughly 70% less manual scheduling work, per the company's own materials as cited by Home Health Care News — sit at the center of how the platform is sold to agencies that have spent years losing money to drive time and dispatcher overtime.

The productivity figure matters because home-health reimbursement is largely visit-based. A clinician who can complete one more visit per day, at the same clinical standard, directly lifts the agency's revenue per full-time employee without adding a single hire. The gain comes from the same routing engine — one that processes millions of data points, from traffic patterns to clinician license levels, to assemble visits in sequences that cut windshield time and idle gaps.

Scheduling is where the second number does its damage. A 70% cut in the manual work of building, rebuilding, and rebalancing visit rosters frees dispatchers from the most repetitive slice of their day. Axle's pitch materials argue that those hours can be redirected to exception handling: late hospital discharges, urgent wound-care referrals, and same-day patient no-shows. In an industry where "schedule coordinator" is among the hardest roles to fill, that labor shift is operationally consequential, not just a line item.

The financial signals around the platform reinforce the operational story. Annual net revenue retention sits at 121%, a figure from Axle's own pitch materials; a number that implies existing customers are expanding seat counts or visit volume rather than churning out. In SaaS terms, high NRR plus triple-digit growth is the pattern investors look for when a product is replacing a manual workflow rather than layering on top of one.

F-Prime venture partner Julia McDowell described the platform as delivering "financial and operational improvements almost immediately," with leadership framing the return on investment as "both substantial and rapid." The pitch, in other words, is that agencies don't need a multi-quarter integration to feel the effect.

What the public data doesn't yet show is the granularity beneath those averages. There is no published breakdown of how the 30% productivity gain splits between reduced drive time, shorter visit turnover, and fewer canceled appointments. There is no third-party benchmark comparing Axle-routed clinicians against a control group on miles driven per visit or overtime hours per pay period. The numbers that exist are company-reported, drawn from the same release that announced the funding round.

Zero G Talent's board currently lists Axle at 15 salaried roles with a median pay band around $200,000, a hiring footprint that hints at where the next phase of operational scale-up will land: more deployment strategists and product engineers to push the routing engine into more agencies, not more clinicians on Axle's own payroll.

How the Market Is Responding

Axle Health's $10M Series A arrived into a home-health software market that had already stopped watching from the sidelines. The broader category was projected to grow from $11.47 billion in 2024 to $155.71 billion by 2034 by market.us, and incumbents spent the months around the Axle round repositioning themselves rather than waiting to see whether AI logistics was a real category.

The clearest countermove came from WellSky, which already owned back-office plumbing and used October 2024 to acquire Bonafide, a software provider for durable and home medical equipment providers, to extend its reach upstream and downstream of any single home visit. That deal signaled consolidation around the existing installed base of agencies. PointClickCare took a different tack at AHIP 2023, rolling out expanded real-time analytics for health plans, an answer aimed at payers, not providers, and a hint that data, not just scheduling, was becoming the competitive axis. Veradigm completed its roughly $140 million acquisition of AI platform provider ScienceIO in March 2024 — a bet that clinical-grade AI belongs inside the electronic health record, not bolted on as a routing layer.

The newer entrants read the same signal differently. Sprinter Health closed a $55M Series B to push its hybrid in-home and virtual model from 5 states to 18 in roughly a year, targeting 22 states by summer's end and approaching 100,000 in-home visits completed. Earlier-stage players acute-care home health provider DispatchHealth and home care tech company Medically Home merged in March 2025, Business Insider reported.

That positioning matters for how agencies and investors now weigh Axle. Home health agencies held more than 61% of the home healthcare software end-user market in 2024, and clinical management systems accounted for over 37.5% of software-type share, precisely the slice Axle's scheduling-and-routing engine competes for. Cloud-based delivery captured 58.1% of the segment, so any buyer evaluating Axle is implicitly comparing it against incumbent stacks from WellSky, Cerner, Epic, McKesson, Allscripts, PointClickCare, Netsmart, and MatrixCare. Axxess, AlayaCare, and Birdie sit closer to Axle on the scheduling-and-care-coordination axis, while market.us singled out Axle for "in-home workforce logistics and visit coordination."

The early adopter side is where the competitive pressure is starting to show up in agency behavior. If Axle's reported productivity figure (up to 30% gains in clinician output) holds in production, it makes a non-AI scheduling stack harder to defend at renewal. The first-party Zero G Talent listings for Axle show the company hiring across engineering, product, and deployment, including a Lead Deployment Strategist with a $190,000–$240,000 band, the hiring shape of a company expecting more agencies to come knocking, not fewer. With 11,474 Medicare-certified home health agencies in the U.S. as of 2021 and home-health agency services hitting $147.8 billion in growth, the addressable buyer pool is large enough that incumbents have to react, even if privately.

Watch the next quarterly M&A tape. If WellSky, PointClickCare, or Netsmart announce a routing-AI tuck-in before Axle's Series B, the market has decided that logistics is a feature to acquire rather than a category to fight. The harder read will be what Axle's existing customers do at the back end of those contracts: whether 121% net revenue retention holds as agencies push the engine into higher-acuity cases, or whether the routing layer starts to look like the first slice of a platform WellSky-style incumbents would rather own than rent.
