We are a team of community-based Responders, with backgrounds as EMTs, paramedics, and MAs, who, in real-time, collaborate with licensed clinicians via telehealth to engage patients in their homes and ensure their full continuum care needs are met through our hybrid care model. ‍ Ready strives to provide a convenient and affordable solution to reduce health costs while partnering with payers and health systems to deliver more positive health outcomes.
