We are seeking a dedicated and compassionate Registered Dietitian to join our team remotely. As part of our interdisciplinary care team, you will play a critical role in empowering patients to achieve their health goals through personalized nutrition counseling and education. This position focuses on providing patient-centered care in collaboration with our clinicians, health coaches, and other healthcare professionals. There is a preference for a Registered Dietitian licensed in multiple states.

Responsibilities:

Available to work Monday - Friday 9:00 am - 5:00 pm Eastern Time

Practice exceptional medical nutrition therapy and counseling within knownwell

Familiarity with the Nutrition Care Process and adherence to RD standards of care is required for this position.

Patient Care and Counseling

Utilize motivation interviewing and behavior change coaching to support patients in engaging in recommended lifestyle changes to improve outcomes.

Provide individual nutrition counseling tailored to patient goals, cultural preferences, and medical needs to support chronic condition management and disease prevention.

Using shared decision making, develop and implement personalized nutrition care plans, incorporating evidence-based guidelines for primary and metabolic health.

Address patient concerns and questions in an accessible and supportive manner.

Patient Education

Deliver patient-centered evidence based education on dietary guidelines, meal planning, and sustainable health practices.

Support patients in understanding and engaging in recommended lifestyle changes to improve outcomes.

Address patient concerns and questions in an accessible and supportive manner.

Interdisciplinary Collaboration

Work as a part of an interdisciplinary team that includes MD/DO, PharmD, RN, health coaches, and APCs (PA and NP)

Participate in team meetings and case discussions to contribute insights on patient progress and nutrition-related challenges.

Documentation and Compliance

Maintain accurate and timely documentation of patient interactions, care plans, and recommendations in the EHR system.

Adhere to clinical policies, HIPAA regulations, and standards of care.

Professional Development

Stay current with developments in nutrition science, primary care, and metabolic health.

Engage in ongoing training and professional development opportunities.

Stay current with licensure and appropriate re-certifications

Requirements:

Active Registered Dietitian (RD) with licenses in alll of the following states: MA, NY, GA, IL, TX,and willingness to obtain additional state licenses as requested by knownwell.

Must be registered with the Commission onboard Dietetic Registration

Bachelor’s or Master’s degree in Nutrition, Dietetics, or related field.

At least 2 years of clinical or counseling experience, preferably in primary care, metabolic health, or weight management.

Excellent communication and interpersonal skills.

High emotional intelligence with the ability to make a patient feel at ease – without judgment and without shame.

Willingness to try new technologies and workflows that are both patient and provider oriented. .

High-speed internet and phone access to support a fully remote practice.
